The Erewash Primary Care Network is working with Erewash Voluntary Action to take forward Social Prescribing in the area. Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

If you are a self-motivated, dedicated ‘people-person’ whose passion is to support and empower individuals to take control of their health and wellbeing, you could be just what we are looking for.

If you are a creative, innovative thinker, who would love the challenge of working across exciting new partnerships within the Primary Care Network and working collaboratively with NHS and Voluntary Sector colleagues in Erewash, this could be the job for you.

The post will be hosted by Erewash Voluntary Action.
