GP LINK WORKER

The Erewash Health Partnership is a collaboration of the 13 GP practices in Erewash working together  and  is committed to improving the care of patients in Erewash.

Erewash Health Partnership is  working with Erewash Voluntary Action to further develop Social Prescribing in the area. Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

If you are a self-motivated, dedicated ‘people-person’ whose passion is to support and empower individuals to take control of their health and wellbeing, you could be just what we are looking for.

If you are a creative, innovative thinker, who would love the challenge of working across exciting new partnerships within the Health Partnership and working collaboratively with NHS and Voluntary Sector colleagues in Erewash, this could be the job for you.

The post will be hosted by Erewash Voluntary Action.

You will adopt an holistic, non-medical, person-centred approach to your caseload, co-designing action plans to improve health and wellbeing based on ‘what matters’ to the person and their individual strengths and needs – and supporting them to achieve their goals.

Ideally you will have worked in a similar role before, but this position would also be suited to someone with good local knowledge, appropriate experience and enthusiasm. If you are proactive, enthusiastic, person-centred and hold a clean driving licence with unlimited access to a car, then we would love to hear from you.

MENTAL HEALTH FORUM ADMINISTRATOR

Erewash Voluntary Action are actively recruiting for an exciting role that will support the Derbyshire Mental Health Forum and the Mental Health Policy Officer.

 

This is an opportunity for someone to join our team as the Mental Health Forum Administrator

The role will be to:

  1. Work closely with the Mental Health Policy Officer by providing administrative support.

  2. Assisting with the setting up and hosting of the Forum meetings – Online and Face to Face

  3. Maintain and develop the membership.

  4. Producing Forum news e bulletins and distributing to relevant parties

  5. Taking minutes for the forum and other relevant meetings

                                                       

18 Hours per week to be worked flexibly.

Salary: £14.00 per hour

SENIOR DEVELOPMENT WORKER – MENTAL HEALTH CRISIS ALTERNATIVES

This is an exciting new role and the successful candidate will be instrumental in supporting communities to provide an alternative to crisis support. This is a role that requires an understanding of mental health challenges and would be an opportunity to work at a senior level to develop new services across Derby and Derbyshire. We require someone with excellent interpersonal skills, a track record in community development and some knowledge of the mental health voluntary sector would be desirable.

37 hours, including some weekend and evening working

Salary: £31.746.00 -  12 month contract (possible extension subject to external funding and review)

We need a new senior development worker who will support the development of pathways for supporting delivery of Crisis alternatives across a wide range of stakeholders.

You will need excellent communication skills, and an aptitude for learning. You must be self motivated and able to work independently within an agreed framework. You will be self-supporting so must be fully computer literate. You will need to be able to work flexible hours and to travel across Derby and Derbyshire.  Posts will be based at our office in Long Eaton.  

The post is initially for a one-year contract (possible extension subject to external funding and review)
